Hydrogel, silver, VAC, enzymatic debrider, hyperbaric oxygen therapy, Regranex and Promogran, collagen-based dressings and synthetic skin substitute graft.

Diagnosis
Pressure ulcer complicated by severe PVD and brittle diabetes.


1-14-04

Wound measurements:

4.5cm x 4.5cm x 0.4cm with
10 % slough.
Initiation of CellerateRX gel.
Covered with a semi-occlusive dressing.


1-21-04

Wound measurements:

Slough completely removed.
Beefy red granular tissue present.
Continued with CellerateRX gel.



3-10-04

Wound measurements:

3.1cm x 3.0cm x 0.2cm.
Continued with CellerateRX gel.


4-22-04

Wound measurements:

2.8cm x 3.5cm x 0.1cm, 100% granular tissue.
Dermagraft placed 7 days prior with little difference in wound.
Continued with CellerateRX gel.



5-12-04

Wound measurements:

1cm x 2.5cm x 0.1 cm.
Healthy pink granular tissue and epithelial tissue visible at edges. Second Dermagraft placed with the use of CellerateRX under the graft and around edges of graft.


6-9-04

Ulcer closed by reepithelialization.



Conclusion: The use of CellerateRX achieved complete closure of this complex chronic non-healing pressure wound without complication in a timely manner.
